We are a growing company that is looking for an Outreach Coordinator to join our team. This is a work from home position. We empower patients to maintain their wellbeing in the comfort and familiarity of their own home. We ensure our patients have the means to obtain medications, understand what medications to take and know how to take them and avoid unnecessary ER visits and hospitalizations. We coordinate care with the patient’s physician to make sure all the patients’ needs are met. This is one of the most rewarding nursing fields you will find; don’t miss your calling! This position is a telephonic, 8 to 5 M-F work from home position.

Responsibilities of the Outreach Coordinator Include but are Not Limited to:

This position is responsible for calling, gaining consent and enrolling patients into our services. Our main goal is to help the patients live their best lives in a safe home environment and prevent hospitalizations through education and support. Must be able to work independently. Must be proficient with self-scheduling, and independently able to maintain a weekly schedule.

Typical Duties of the Outreach Coordinator Include:

· Performs calls to patients to educate patient on the program and explain the benefits to patients of participating in the program.

·Enrolls patients into the program through HER.

· Identifies any immediate needs the patient has on that call and provides information to the appropriate staff.

· Collaborate with other members of the “care team” including patient’s physicians and other medical professionals.

· Updates EMR and spreadsheets for calls made and follow process for unable to contact patients.

· Sends a daily email to manager on outcomes of daily enrollment.

· Works as a team to continue to keep a positive professional and productive work environment.

· Completion of required paperwork and documentation within mandated time frame.

· Maintaining professional outreach coordinator responsibilities by setting appropriate boundaries, complying with expectations and productivity standards, and seeking managerial consult, as needed.

· Comply with all on-boarding, annual and other mandatory training as assigned.

· Maintain HIPPA compliance.

· Performs other related duties as assigned by management.

Requirements of the Outreach Coordinator:

· Computer skills required: Excel, Microsoft Office, Microsoft Word, and Outlook, preferred.

· Must have intermediate computer skills and proficiency with Microsoft Word, Outlook and Excel.

· Healthcare related background preferred but not required.

· Excellent Verbal Communication and Phone skills.

· Able to multi-task with proficiency.

· Ability to prioritize multiple tasks.

· Ability to represent themselves in a professional manner.

· Must have a valid driver's license, and a smartphone.

· High Speed Internet Connectivity.

· Must have own computer

Job Type: Full-time

Pay: $12.00 - $15.00 per hour
